Retreat Registration Deadline is July 13, 2012.

Please read the information below and complete the registration form. The registration form is an editable pdf file. Please type in the requested information, including all applicable account information, print the document, ask your PI to sign it, and return the form to Lucita Nacionales or Pat Veitch (Box 2811, Room RH-484A) or email to lucita.nacionales@ucsf.edu

Important Information Regarding the Neuroscience Retreat:

1. You must return the registration form to Lucita Nacionales or Pat Veitch by July 13.
Any registration forms received after July 13 will be subject to a $50.00 late registration fee.
The late fee will be charged to your PI's account.

2. Cancellations received after July 20 cannot be refunded.
This means that your PI will be charged for retreat costs as if you attended the conference. Asilomar charges us for the number of attendees that we provide them on July 20, so, we cannot provide refunds after this date.

3. Roommate information.
We will do our best to place you in a room with the person whom you designate as your preferred roommate (please be sure to tell your preferred roommate(s) that you wish to room with them in case they have already made arrangements to room with others). Please feel free to list a second preferred roommate as a backup in case your first choice is not available or if you wish to be placed in a 3-person room. If you do not indicate a preference we will assign you a roommate.

Please note: Due to the various room configurations at Asilomar we cannot guarantee the number of roommates that you will have. Some of the rooms provided may accommodate 3 or 4 people, so, even if you only request one roommate you may be placed in a room with 2 or 3 others. Thank you for your understanding.

4. Meals.
Please indicate if you would like a vegetarian meal or non vegetarian meal by checking YES or NO on the registration form. If you do not indicate a preference a non-vegetarian meal will be provided.

5. T-Shirt.
Please indicate your t-shirt size on the registration form. If you do not want a t-shirt, please select the no shirt box.

6. Retreat Cost.
The retreat will cost approximately $400 per person. The cost per person is calculated by dividing total cost by the total number of participants. We will recharge after the retreat. Neuroscience students and faculty who attend the full retreat will be charged no more than $200. Students and faculty will have to sign into the first session and out of the last session to obtain this discount.
